We find the article by Baik et al.1 fascinating, but to be updated, considering the inherited real risk (IRR) of coronary artery disease (CAD), conditio sine qua non of CAD, described in detail previously.2, 3, 4, 5, 6 In our opinion, it is firstly unavoidable to divide CAD risk factors into two groups:

(A) Environmental risk factors (∼300).
